The “FREE ONLINE PAIN EVALUATION” tool is located on the sidebar area to your right. Please open each of the tabs located at the top of the tool. There are three tabs after the initial “default” START tab. Please fill in all appropriate areas as best as you can to ensure that your evaluation is given the best attention possible. When you have finished filling in all of the blanks on all three of the tabs click SEND at the bottom of the SEND tab (tab 3).
We will immediately start gathering the appropriate information to help you get the best initial pain evaluation as soon as possible.
